Patient Care Reports

The North Kansas City Fire Department maintains records of the medical care that we provide to our patients. Due to HIPPA law, only authorized persons may obtain copies of these records. This includes the specific individual that is named on the patient report, or another person that has a valid, notarized patient release or power of attorney and valid identification.

Patients Requesting Records
Please call the Fire Department 816-274-6025 or by stop by the Fire Station at 1815 Howell Street in North Kansas City, MO 64116. You will need to show your I.D. to get the copy. There is no charge for the record.

Law Firms or Legal Copy Services Requesting Records for a Client
Attorneys requesting copies of patients records must request the records in writing and submit a patient authorization form. Please send correspondence to nkcfd@nkc.org. An invoice will be generated if a record exists. The fee for these records is $12.00. Payment must be received before the request will be processed. Acceptable forms of payment are cash, check or money order.

Fire Victims Requesting Reports
Fire reports may be obtained by victims of fire at no charge. Fire reports may not be immediately available. Please allow at least 24 hours before trying to obtain your fire report. You may pick up your copy at Fire Station 1 at 1815 Howell. A photo I.D. will be required.

Law Firms or Insurance Companies Requesting Fire Reports
Attorneys and insurance companies may obtain a copy of a fire report upon receipt of a written request and payment of $12.00 made payable to the City of North Kansas City. Written requests may be faxed, emailed, or sent by mail, however payment must be received before the request will be processed. Acceptable forms of payment are cash, check or money order.
